GREEN SHEET: Due Dates for Winter Quarter 2010
Application must be completed during pre-registration or before the end of the 1st week of school to be considered "officially enrolled."

REGISTRATION
- Complete and submit a Co-op application to Co-op office
ORIENTATION
- Download a Co-op Student Handbook
- Read and understand the Student Handbook
- Complete objective worksheet and have it approved by your supervisor
- Provide copy of your completed objective's worksheet to your instructor - can be e-mailed or submit hard copy
REQUIREMENTS
- INTERNATIONAL STUDENTS: PAID INTERNSHIPS - Employment letters must be submitted with your application.
- VOLUNTEER COMMUNITY SERVICE - volunteer letter must be submitted with your application
RESPOND TO E-MAIL SENT TO YOU AND CALL YOUR INSTRUCTOR

SECOND WEEK JANUARY 11 - 15
|
- MEET with your Co-op instructor and submit completed objective worksheet if not done so already
- Set up an appointment for a student/instructor work-site visit
- Discuss a midterm project with your instructor.
- If you are not able to complete objectives for week 1 and 2, you are advice to speak with your instructor ASAP or drop the course

THIRD WEEK JANUARY 18 - 22
|
- Meet with instructor and supervisor at the worksite
- Ensure Performance Agreement is completed and signed

SEVENTH WEEK
FEBRUARY 1 - 5
DUE: FEB. 3
|
- Submit completed project & 1ST TIME CARD to instructor
- Instructor will provide feedback on quality of the written report
- If you are not able to complete these tasks by now, you must drop the course OR receive a failing grade

SEVENTH WEEK FEBRUARY 15 - 19
|
- COMMUNICATE WITH INSTRUCTOR AND PROVIDE A BRIEF PROGRESS REPORT

NINTH WEEK MARCH 1 - 5
DUE: MARCH 3
|
- E-MAIL your Self-evaluation to your instructor. (Your evaluation is a two- to three-page written report explaining the process that you took to complete your goals and the learning outcome - see criteria)
- Submit last two time cards (hours worked for the last two months of the quarter)
- Instructor will provide feedback on your report.
- You will have an opportunity to resubmit self-evalution for credit if needed
- Call your instructor to provide brief progress report

- Re-submit your self-evaluation if needed
- Course ends
