How to Apply for Deaf Services

Become a De Anza College Student
Submit a DSPS Application
Get an Initial Intake Appointment with a DSPS Counselor
  1. Register as a De Anza College Student at: www.deanza.edu/registration/myinfo.html 
  2. Use your CWID (CampusWide Identification #) and password to login to the De Anza College "MyPortal" site at: https://myportal.fhda.edu/cp/home/displaylogin 
  3. Complete and submit the DSPS Application for services
    • Select the "Students" tab and find the "ClockWork Student Portlet"
    • Select the "ClockWork De Anza Disability Progs & Svcs"
    • Select "DSPS Application

Complete the 3 steps and you will promptly be contacted by a DSPS staff member to set-up an appointment to meet with a DSPS Counselor to arrange your services.

During the DSPS Application process, you will be asked to provide some disability verification documentation. If you have these documents in a digital format (pdf, doc, docx, txt, rtf, xls, xlsx, ppt, pptx, wpd, zip) you can upload them directly to DSPS during the application process.


If you are unable to upload the documents you can send them to DSPS:

  1. By fax at (408) 864-5492 
  2. Via email at dss@fhda.edu
  3. Bring a hard copy into the DSS office on the main campus in the Sutdent and Community Services Bldg. (SCS) Room 141


An audiogram is required in order to qualify for Interpreting/Captioning Services. Please be ready to upload a copy of your audiogram signed off by your medical provider.

  • It has to be within the last three years.
  • If it is more than 3 years ago, you must have a new audiogram.
