Application Process

1. Check the eligibility requirements for EOPS.
2. If you feel that you may be eligible, contact EOPS or check this web page for the application period for each quarter.
3. Download and complete the following forms and submit in person to EOPS during the application period:
   • EOPS/CARE Application Form
   • Mutual Responsibility Contract
   • BOG (Board of Governors Fee Waiver)
4. If you are not currently a student at De Anza, you should also:
   • Apply for De Anza admission.
   • Take the De Anza Math and English or ESL placement tests.
