Instructor Skills Referral Form

Instructors who refer students to skills classes can use this form to specify the skills areas that need improvement.

Mac Users: Click and hold down on the document link you want to download. Choose Save As, Save Link As or Save (depends on browser and version). Then, specify the location you want to save the document in and click Save.

PC Users: Right click on the document link you want to download. Choose Save As, Save Link As or Save (depends on browser and version). Then, specify the location you want to save the document in and click Save.

Download the Instructor Referral Form (PDF) then print it out, complete it, and have the student bring it to room L47 to sign up for a skills class.
