Health Requirements TB and MMR: Measles, Mumps and Rubella

  • Time: 9am - 4pm
Students who are studying from INSIDE the U.S. are required to visit the De Anza Health Office first week of classes January 8 - 11 in person to schedule an appointment for either January 23 or 24 to review TB & MMR health requirements. De Anza Health Service is located in the Hinson Campus Center, Lower level.

Please note:
  • Remember to make your appointment in person first week of classes to receive a number for your January 23 or 24 appointment slot.

  • DO NOT upload any medical document to your Health Portal PyraMed until after your appointment!

  • DO NOT email medical documents as they will be automatically deleted due to privacy reasons.

  • Students must be registered for current quarter and paid the health fee.
