One-Question Survey on COVID Vaccination Requirement
Dear Student,
We’d like to get your input on the COVID-19 vaccination requirement that's currently in place for students and employees who come to De Anza’s campus in person.
Please answer this one-question survey about whether the Foothill-De Anza Community College District should keep the current requirement or end the requirement as of April 10, when classes begin for spring quarter.
Please complete the survey no later than noon on Thursday, Feb. 23. Results will be considered by the district Chancellor’s Advisory Council and the Board of Trustees.
