2006 California Chemistry Diagnostic Exam Score Transfer Form

A score of 22 or higher out of 44 on the 2006 California Chemistry Diagnostic Exam is required to earn placement into CHEM1A and BIO6A.

What you should submit:

  1. This completed form

  2. Email it to daassessment@deanza.edu 

Note: We can only accept scores earned from the 2006 California Chemistry Diagnostic Exam. We cannot accept any other scores, results or clearances earned from other sources.
