The Psychological Service department is opened for Fall quarter. If you would like to make a first time appointment for Fall quarter you may download and complete the required intake forms & send them to email@example.com. Please include your name, CWID, availability, & attach the three intake forms. You can find these forms under our make an appointment tab to the left of the page. If you're a returning client, we would like you to complete the same process to have updated information. We thank you for your time and patience.
